What It Does?

Medicine mainly used in fungal infections of the skin and in the vagina in women when treatment with medicines that you put on your skin or medicines that you insert into your vagina has not helped. The medicine can also be used to treat a particular type of meningitis and other types of fungal infections of man is immunocompromised.

Diflucan is indicated for the treatment of the following fungal infections (see section Pharmacodynamics).

Diflucan is indicated in adults for the treatment of:

  • Cryptococcal meningitis (see Warnings and Precautions).
  • Coccidioidomycosis (see Warnings and Precautions).
  • Invasive candidiasis.
  • Mucosal candida infections including oropharyngeal and esophageal candidiasis, candiduria and chronic mucocutaneous candidiasis.
  • Chronic oral atrophic candidiasis (protesstomatit) on oral hygiene and topical treatments are inadequate.
  • Vaginal candidiasis, acute or recurrent; when local therapy is not appropriate.
  • Candidabalanit when local therapy is not appropriate.
  • Dermatomycosis, including tinea pedis, tinea corporis, tinea cruris, tinea versicolor and candidadermatit when systemic treatment is indicated.
  • Tinea unguinium (onychomycosis) when other agents are not considered appropriate.

    Diflucan is prescribed for adults to prevent:

  • Relapse of cryptococcal meningitis in patients with high risk of recurrence.
  • Relapse of oropharyngeal or oesophageal candidiasis in patients with HIV infection who are at high risk of relapse.
  • To reduce the incidence of recurrent vaginal candidiasis (4 or more episodes a year).
  • Prevention of fungal infections in patients with prolonged neutropenia (such as patients with haematological malignancies during chemotherapy or patients receiving hematopoietic stem cell transplantation.

    Diflucan is intended for neonates, infants, toddlers, children and adolescents aged 0 to 17 years:

    Diflucan is used for the treatment of mucosal candidiasis (oropharyngeal or oesophageal), invasive candidiasis, cryptococcal meningitis, and prevention of fungal infection in immunocompromised patients. Diflucan can be used as maintenance therapy to prevent relapse of cryptococcal meningitis in children with high risk of recurrence (see Warnings and Precautions).

    Treatment can be started before the cultures and other laboratory tests are completed, but as soon as the results of these are available, the anti-infective therapy adjusted accordingly. Official guidance on the appropriate use of antifungal agents should be considered.

    Patients on regular dialysis should receive 100% of the recommended dose after each dialysis. Days without dialysis patients should receive a reduced dose adapted to the patient's creatinine clearance.

    There are limited data in patients with hepatic impairment, therefore fluconazole should be administered with caution to patients with liver disorders (see Warnings and Precautions and Undesirable Effects).

    A maximum dose of 400 mg daily should not be exceeded in the pediatric population. As for similar infections in adults, the duration of treatment based on the clinical and mycological response. Diflucan given as a daily dose.

    For pediatric patients with impaired renal function, see dosing in "Renal impairment". The pharmacokinetics of fluconazole has not been studied in children with renal insufficiency.

    How It Works?

    The active ingredient in the medication attacks the fungal cell wall so that the fungus cannot grow.

    How It Is Taken?

    The dose of Diflucan is determined based on the type and severity of the current fungal infection. Treatment of infections requiring multiple dosing should be continued until clinical parameters or laboratory tests indicate that the active fungal infection has subsided. An insufficient treatment period may lead to recurrence of active infection.

    Dosage should be adjusted in mature patients according to renal function.

    No adjustments are needed in treatment with a single dose. In patients (including pediatric population) with impaired renal function who will receive multiple doses of fluconazole should be given an initial dose of 50 mg to 400 mg, based on the recommended daily dose for that indication. After this initial loading dose, the daily dose (according to indication) based on the following table.

    Is It For Me?

    Hypersensitivity to the active substance, to related azole compounds or to any of the excipients.

    Coadministration of terfenadine is contraindicated in patients receiving Diflucan at multiple doses of 400 mg per day or more based on the result of an interaction study with repeated doses. The concomitant administration of other drugs known to prolong the QT interval and which are metabolised via the cytochrome P450 (CYP) 3A4, including cisapride, astemizole, pimozide, quinidine and erythromycin are contraindicated in patients receiving fluconazole.

    Any Drug Interactions or Incompatibilities?

    Concomitant administration of fluconazole at doses lower than 400 mg per day with terfenadine should be carefully monitored (see Contraindications and Interactions).

    If you take several medications at the same time, there is a risk that they influence each other. It allows, for example, may need to change the dosage.
