Adults and children who are referred for patch testing of allergens appear equally likely to have allergic contact dermatitis, although they tend to react to different allergens, according to a new report.

Skin reactions to allergens are common among children, according to background information in the article. They can occur both on areas of the skin that come in direct contact with an allergen (contact dermatitis) and on areas that aren’t directly affected (atopic dermatitis). Children suspected of contact dermatitis are often referred for patch testing, in which skin is exposed to various allergens affixed to a plaster tape to identify which cause a reaction.

Kathryn A. Zug, M.D., of Dartmouth-Hitchcock Medical Center, Lebanon, N.H., and colleagues analyzed results from 391 children age 18 and younger who underwent patch testing between 2001 and 2004. They compared results of the pediatric population to a group of 9,670 adults (age 19 and older) who were tested during the same time period.

Children and adults tested positive for at least one allergen at approximately the same rate (51.2 percent for children vs. 54.1 percent for adults). “Our study showed significant differences between the frequency of individual positive reactions to allergen patch tests in children and adults; children were more likely to have reactions to nickel, cobalt, thimerosal and lanolin, whereas adults were more likely to have positive reactions to neomycin, fragrance mix, M. pereirae (balsam of Peru [an extract from the balsam tree used as an alternative therapy]) and quaternium 15,” the authors write.

Some children reacted to supplemental allergens not included in common patch test series (15 percent) or in commercially available tests (39 percent). “Patch testing in children suspected of having allergic contact dermatitis is a valuable endeavor. Despite their limited back size, an expanded allergen series helps to identify important positive relevant allergens. Allergen concentration does not need modification for testing in children,” the authors conclude. “The top 45 allergens with the most frequent positive and relevant reactions reported in this study should serve as a guide to patch testing in children suspected of having allergic contact dermatitis in North America. Including supplemental allergens to the patch test materials based on clinical suspicion is also useful in some patients.“

This study was supported by an American Contact Dermatitis Society Clinical Research Award the Nethercott Award for the study of epidemiology of contact dermatitis.

Molecules from cone snail venom and African plants are being used by Queensland researchers as a blueprint to develop an oral drug to treat chronic pain.

Professor David Craik and Dr Richard Clark from the Institute for Molecular Bioscience have received $218,275 from the National Health and Medical Research Council (NHMRC) to aid in translating their research into a product available for Australians to use.

Studies on the molecule they have developed have shown that it is effective in relieving neuropathic pain in animals.

“Neuropathic pain is one of the most severe forms of chronic pain, and very difficult to treat,” Dr Clark said.

“Regular pain occurs when the nervous system is stimulated by, for example, an injury, whereas neuropathic pain occurs when the nervous system itself is damaged.”

“Current treatments in neuropathic pain only provide meaningful relief for one in three patients, and all of the current market-leading drugs have serious side effects, as well as taking up to three weeks to begin to take effect.”

Peptides (small proteins) from cone snail venom have attracted recent attention from scientists, as they can target receptors with a high degree of accuracy, thus eliminating severe side effects.

But peptides also degrade rapidly in the body. Professor Craik and Dr Clark have overcome this problem by engineering a circular peptide, using a circular protein backbone discovered by Professor Craik and found in plants such as violets.

The NHMRC Development grant will allow the researchers to further test their molecule to fully establish its therapeutic potential.

“Successful outcomes from this project will provide additional confirmation of the suitability of our molecule as a treatment for neuropathic pain,” Dr Clark said.

“Armed with these data, we will be able to secure a commercial partner and develop this molecule into a tablet for sufferers of chronic pain.”

Genital herpes is estimated to infect about 20 percent of the population, with many not really knowing they have been infected by the virus. Glucocorticoids in high dosage is the accepted treatment for severe dermatomyositis-polymyositis, though there is no controlled trial to prove its effectiveness. Prednisone is generally started at a dose of 1 to 2 mg/kg body weight per day (60 to 100 mg/d for adults). Improvement may begin within 1 to 4 weeks, though in some patients treatment may need to be continued for 3 months before improvement occurs. When improvement is noted, the daily dose may be reduced by 5 mg every 4 weeks. Adults with acute to subacute dermatomyositis-polymyositis tend to improve more rapidly than those with chronic polymyositis; children also respond in most cases. If the dose is reduced too rapidly, or to too low a level, relapse will occur, necessitating return to high dosage. Prednisone therapy may have to be continued for several years, but an attempt should be made every year to withdraw the therapy from patients who are clinically stable in order to determine if the disease is still active.

In a small controlled study, high-dose intravenous immunoglobulin (IVIg) therapy (0.4 g/kg daily for 5 consecutive days) produced improvement in patients with steroid-resistant dermatomyositis and probably should be used in severe or refractory cases. Anecdotal reports have suggested that IVIg may also be effective in polymyositis resistant to other forms of therapy. Total-body irradiation has been successfully used in some patients with disease refractory to glucocorticoids and immunosuppressants, but long-term follow-up and controlled studies are lacking for this potentially dangerous therapy. A controlled trial of plasma exchange and leukapheresis in a small number of patients with dermatomyositis-polymyositis resistant to glucocorticoids showed no benefit over sham pheresis. Bed rest has been recommended in the acute phase of the disease but is harmful in the long term. Physiotherapy and rehabilitative devices are important in the long-term treatment of patients with dermatomyositis-polymyositis.
